CalViva Health Medi-Cal New Provider Resources

CalViva Health is a licensed health plan that contracts with Health Net to serve Medi-Cal enrollees in Fresno, Kings and Madera counties. CalViva Health and Health Net Community Solutions, Inc. (Health Net) are excited about the opportunity to partner with you to meet the health care needs of Medi-Cal beneficiaries. The following materials and the trainings available online supplement the Plan's operations manuals to assist you in understanding and adhering to policies and procedures in accordance with your Provider Participation Agreement (PPA). In-person training is also available. • Better Communication, Better Care: Provider Tools to Care for Diverse Populations toolkit (PDF)
  This toolkit is produced by the Industry Collaboration Effort (ICE). ICE is a multidisciplinary team of providers, health plans, associations, and accrediting bodies that work collaboratively to implement and improve regulatory compliance. The toolkit includes provider tools and resources to care for and interact with diverse populations.
• Medi-Cal Member Rights and Responsibilities flyer (PDF)

• Certificate of New Provider Training form (PDF)
  Physicians and other providers must sign the form to attest that they have received the Plan's required Medi-Cal trainings.
