Unhoused in Central Sierra Region Receive Resources Through Health Net's $162,000 Investment

Continuums of Care are groups that plan and organize housing and services for unhoused individuals and families. The Central Sierra Continuum of Care (CS CoC) is key to making sure local housing programs run smoothly, securing funding from state and federal programs and bringing community partners together.

In line with our commitment to whole-person care, Health Net is awarding CS CoC a $162,000 grant. This funding will enable CS CoC to recruit staff responsible for meeting federal and state compliance standards, reinforcing collaborations and advancing programs like Enhanced Care Management and Community Supports under California's CalAIM framework.

CalAIM is California's program designed to transform Medi-Cal by addressing both health and social needs, especially for the state's most vulnerable people. Programs like Enhanced Care Management and Community Supports rely on strong partnerships between health plans and housing providers, as well as accurate data to coordinate care.

That's where Health Net's investment makes a difference. The funding will also help improve the Homeless Management Information System, a secure database that tracks services for unhoused people.

By strengthening organizational capacity, data systems and cross-sector collaboration, this investment aims to achieve measurable improvements in health outcomes, housing stability and service coordination for individuals and families experiencing homelessness.
