Introduction
Health Net, LLC ("Health Net") is pleased to announce a Request for Proposal (RFP) for the Community & Infrastructure Investment Program. Funding for this 2021 Enhanced Access RFP will focus on Enrollment, Retention and Education for California’s diverse health care consumers. The purpose of this grant program is to assist consumers with enrolling in and retaining health coverage (including Medi-Cal and Medicare), understanding their benefits and navigating the health care system.
Funding
Pursuant to this competitive RFP, Health Net will fund up to $1.8 million to organizations in California dedicated to strengthening local and regional efforts in the following:
- Enrollment and renewal assistance
- Enrollee support/navigation
- Reduction of barriers to health coverage
- Outreach and education on health coverage
Funded program activities shall:
- Inform consumers of availability and benefits of health care coverage
- Facilitate eligible consumers in health care coverage enrollment
- Assist consumers with the health coverage renewal process
- Provide extensive enrollee support and navigation post-enrollment
- Reduce barriers to health coverage
Funding terms will be 13 months, with a maximum grant award of $100,000 per applicant. Award amounts will be determined based on project scale and current active Enhanced Access grants (if applicable).
COVID-19 Pandemic Impact
The COVID-19 pandemic and economic crises have had significant impact on the healthcare delivery system. Many healthcare organizations have experienced challenges and interruptions forcing them to shift from direct in-person practices to remote services to provide access to care. Additionally, the state of California is facing an influx of newly eligible Medi-Cal consumers who will need assistance enrolling, retaining and understanding how to navigate within the Medi-Cal healthcare delivery system.
Health Net is committed to providing care for California’s most vulnerable populations. We understand that the COVID-19 pandemic has required safety net organizations to adjust many of their traditional methods of providing enrollment, retention and education services, and we are committed to supporting the continuation of services during these unprecedented times. In response, we are increasing the number of grants traditionally awarded for this initiative, and expanding the length of our grant program to 13 months.
We are interested in learning about creative solutions implemented in response to the pandemic to provide access to care. In the online application section titled COVID-19 Pandemic: Impact and Response, please describe how the pandemic has impacted your organization, and the new strategies your organization developed in response (e.g. modifications to programs and operations, shifts in the clients served, adjustments to budgets and fundraising plans, etc.).
Eligibility
Eligible organizations must have:
- Experience providing outreach and health access for communities facing health disparities based on factors including race, ethnicity, color, gender, national origin, religion, language, sexual orientation, gender identity, disability status, homelessness or veteran status
- Primary leadership from among the people served
- Proven success enrolling and retaining consumers in health care programs
Organizations who have an active Health Net Enhanced Access grant under a no-cost extension are eligible to apply for pro-rated funding. Please include specific details in your application to address your active grant progress to date and how you plan to continue or enhance your work.
Applicants must submit proposals that:
- Demonstrate prior success in helping consumers enroll and retain coverage, or in providing enrollee support/navigation
- Include collaborations – or detailed plans to collaborate – with other community based organizations to achieve higher consumer enrollment and retention rates
- Hire and maintain staff from within the community served
Additional consideration will be given to organizations with the following:
- Proposals with emphasis on retention rates
- Proposals with a regional approach to enrollment, retention and navigation
- Proposals with creative outreach solutions due to the COVID-19 pandemic
- Proposals that build upon work conducted under previous grants funded by Health Net
Please note: If Applicant is awarded a grant and serves as a Certified Enrollment Entity or Navigator funded by the California Health Benefit Exchange (Covered California), Applicant must fully comply with any applicable state and federal statutes, regulations, and sub-regulatory guidance requiring disclosure of receipt of funding to Covered California and consumers receiving application assistance from Grantee's personnel. At the instruction of Covered California, awardees may not use any portion of their Health Net grant for assistance with enrollment in any Qualified Health Plan.

During the online application process, you will be required to provide the following documents:
- Proposal Cover Letter: A letter on the organization's letterhead that includes, at a minimum:
- The legal name of the organization
- The organization's street address as listed in the application
- Signature of and date signed by the chief executive of the organization
- Brief description of proposed program and requested funding amount
- Board of Directors: Names of the applicant's Officers and Directors, and/or managers, and include officer/manager positions and their affiliations
- IRS Form W-9: A signed copy. The form for your fiscal agent will suffice if applicable
- Organizational Budget: Please attach a copy of your organization's current itemized operating budget
- Accounts Payable ACH Form (for electronic deposit): If awarded, payment will be electronically deposited into your organization's designated bank account through ACH. An ACH payment remittance advice will be delivered via the email address specified on the form. The form is to be completed by the requesting organization and must contain the legal name of the organization and the signature of a company authorized individual.
