General Agent Incentive Terms and Conditions

Program Guidelines

• Group/Membership Eligibility:
  • Eligibility for this promotion is limited to California (CA) Small Group new sales only.
  • A new CA Small Group consists of 1-100 employees; groups and members must be active through December 31, 2019.
  • To qualify, General Agents' sales representative(s) must sell a minimum of 200 new CA Small Group members with effective dates 7/1/19 through 12/1/19.
  • New CA Small Group qualifying members are those who have enrolled in a medical plan with the same enrollment effective date as the employer group's original Health Net effective date (i.e., if an employer group's original effective date is 10/1/19, then the members must also have an effective date of 10/1/19).
  • Exclusions:  Enrollment in dental, vision, prescription drug, life insurance plans, covered by COBRA or other continuation plans, as well as Cal Choice and Covered California Small Group members are not eligible for this promotion.
• General Agent Eligibility
  • Participating General Agents must be in good standing with a current, signed Health Net broker contract and valid license on file.
    • To be credited, General Agent must be the General Agent of Record at the time of the period close.
    • Group splits with another General Agent sales representative for credit purposes are not eligible for this promotion.
    • Sales representatives are employed by General Agents and work with Brokers to bring new business to Health Net within the assigned territory.
    • General Agent must provide reporting as detailed on the other side of this flyer.
• Membership totals for the promotional period based on general agent reporting will be verified by Health Net 60-days after the end of final program period. Membership numbers as of the last day of the qualifying period will be used to compute membership totals and will be considered final.
• Final reward will be delivered by 3/1/20.
• The dollar value of the promotion will be reported via 1099 forms.
• Health Net reserves the right to change or terminate the program at any time.
