  1. FDA Approved Indications:
    • Management of postherpetic neuralgia (PHN)
  2. Health Net Approved Indications and Usage Guidelines:
    • Diagnosis of postherpetic neuralgia (shingles or herpes zoster)
    AND
    • Failure or clinically significant adverse effects to a gabapentin dose greater than or equal to 1,200 mg/day
  3. Coverage is Not Authorized For:
    • Non-FDA approved indications, which are not listed in the Health Net Approved Indications and Usage Guidelines section, unless there is sufficient documentation of efficacy and safety in the published literature.
  4. General Information:
    • Gabapentin has been shown to provide pain relief for post-herpetic neuralgia at dosage of 1200 to 3600 mg/day.
  5. Therapeutic Alternatives:
