The subscriber number can be found on your Health Net Insurance card. Please enter the complete ID, including all letters and numbers. All letters must be typed as capital letters.
Important News from Health Net
Your NJ FamilyCare/Medicaid coverage will be moving from Health Net Healthy Options (Health Net) to AmeriChoice of New Jersey, Inc. (AmeriChoice) on May 1, 2010. This means that AmeriChoice will take care of your health care coverage beginning on May 1, 2010. Health Net and AmeriChoice are working together to make this an easy change for you. There will be no gap in your coverage and no changes to your current benefits.
If you do not want to be an AmeriChoice member, you have the right to contact the State's Health Benefits Coordinator (HBC) to choose another health care plan that participates in your county or, if you qualify, you may be able to select Medicaid Fee-For-Service (FFS). The phone number for the HBC is 1- 866-472-5338 (TTY 1-800-701-0720). You have until April 9, 2010 to select another health care plan that participates in your county or FFS, if applicable. If you do not choose another health plan by April 9, 2010, your health care coverage will be moved to AmeriChoice.
If your coverage is moved to AmeriChoice, you will receive a new member Welcome Letter and Member Handbook from AmeriChoice. Please make sure you review the Handbook when you receive it and contact AmeriChoice if you have questions. By early May, you will receive a new AmeriChoice ID card. Please start using your new ID card on May 1, 2010 or as soon as you receive it. Beginning May 1st, if you need to get care or see a provider and you have not received your AmeriChoice ID card, your provider may contact AmeriChoice at 1-800-941-4647.
You should continue to use your Health Net ID Card and providers through April 30, 2010.
You can check to see if your current PCP is part of the AmeriChoice network by calling AmeriChoice Member Services at 1-800-941-4647 TTY #1-800-852-7897. If your current PCP is part of the AmeriChoice network and you want to keep seeing your PCP, you do not need to do anything. If you want to change your PCP, call AmeriChoice Member Services at 1-800-941-4647 TTY #1-800-852-7897 or the HBC at 1-866-472-5338 (TTY #1-800-701-0720).
If your current PCP is not part of the AmeriChoice network, you may call AmeriChoice to select a new PCP or call the State's HBC at 1-866-472-5338 (TTY 1-800-701-0720) to see if your PCP belongs to another health care plan that participates in your county. If you select a new PCP, ask your old PCP to transfer your Medical Records to your new PCP. Your old PCP must do this at no charge. If you do not select a new PCP and your current PCP is not part of the AmeriChoice network, AmeriChoice will select one for you. Once you become an AmeriChoice member, you can change your PCP.
Just as with Health Net, AmeriChoice members do not need to choose a Primary Care Dentist. You can see any dentist in the AmeriChoice network. If you need help choosing a dentist, please call AmeriChoice at 1-800-941-4647 (TTY# 1-800-852-7897).
Many of Health Net's hospitals are also part of AmeriChoice's network; however, there are three (3) hospitals that are not in AmeriChoice's network. Below are the three (3) hospitals not in AmeriChoice's network and information about nearby hospitals that you can go to that are in AmeriChoice's network. As a reminder, in the case of an emergency you should always go to the closest hospital even if it is not in the AmeriChoice network, or dial 911.

If you are seeing a provider who is in the AmeriChoice network, you can continue to see this provider without calling AmeriChoice. Some services may require your provider to call AmeriChoice to obtain approval. If you have been approved for services by Health Net, you may continue receiving these services under AmeriChoice until your approval expires.
If you are being treated by a provider who does not participate with AmeriChoice, you have the right to continue seeing this provider for up to four months after May 1, 2010 or through August 31, 2010. If you are seeing a provider who is not in the AmeriChoice network, you should call AmeriChoice Member Services. Member Services will help change your provider to a provider in AmeriChoice's network for services after September 1, 2010. If you are currently seeing a provider who is not in the AmeriChoice network for one of the following conditions, you will be able to continue treatment as follows:
If you are enrolled in a Health Net care management program, all of your information, including your care plan, will be reviewed with an AmeriChoice Care Manager. An AmeriChoice Care Manager will call you to discuss your current plan and help you with any additional needs or changes.
Most of the pharmacies available to Health Net members will be available to you as an AmeriChoice member. If your pharmacy is not in AmeriChoice's network, you will receive information about other pharmacies in your area and how to switch your prescriptions to one of these pharmacies. Your pharmacy benefit will stay the same with AmeriChoice. You will continue to receive the medicines that you are currently taking. If you receive your medicines with your Medicaid card, none of these changes will apply; you will continue to get your medicines by using your Medicaid card.
