Small Group POS Plans
Health Net of Arizona offers a variety of health plans and access to quality health care for most group sizes. The depth of our products and our quality customer service are well equipped to meet your needs.
Health Net of Arizona's POS plans allow members to receive most care without an authorization or referral from a physician. When medical care is needed, they simply call a Health Net POS physician and schedule an appointment. POS plans offer a choice of accessing services in-network and out-of-network. Physicians and facilities contracted with the Health Net POS network are referred to as in-network or network providers. Out-of-pocket costs are likely reduced when selecting in-network providers.
POS plan members are not required to select a Primary Care Physician (PCP), however we encourage all members to establish a relationship with a family physician who can become familiar with their medical history and help them make appropriate medical decisions.
POS plan members may be billed for the deductible, copayment or coinsurance for covered services or required to pay estimated out-of-pocket expenses at the time of service.
When visiting a provider that is not within the Health Net POS network, members may also be required to pay for charges that exceed eligible expenses for covered services.
Certain services, including hospital and outpatient surgery, require precertification. For more information about these specific services, call the Customer Contact Center. You should also call to confirm if precertifcation has been obtained by your doctor when required.

* In Arizona, PPO/POS members living in Apache, Navajo, Yavapai and Yuma counties must always use the First Health network when accessing care in these areas and outside of Arizona. When accessing care in all other counties in Arizona, PPO/POS members must use the Health Net provider network.
